All documents must be signed by either a parent or legal guardian. All references to "participant" shall be deemed to include the parent or legal guardian of such participant who is a minor.

Participant desires to take part in Camp Valle Verde, in the session and dates indicated above. As a condition to participation, he/she agrees to the following:

Participant acknowledges that a wide variety of activities may be conducted at camp. Participant realizes that some of these events may subject him/her to certain stresses and hazards, not all of which can be foreseen. Participant desires and consents to take part in all such events and activities (except when requested to be excluded for medical or other reasons). Participant assumes all of the ordinary risks normally inherent to the nature of the activities and events to be conducted and agree that neither Camp Valle Verde (Formación y Reacreación, S. C.) nor any of its representatives or other persons conducting such activities shall be responsible for any damages or injuries resulting to participant in the absence of evidence of gross negligence.

As a part hereof, participant has read the Code of Conduct (below) containing rules and regulations which all participants are expected to follow and obey. Participant acknowledges having read such code, recognizes its need and agrees to comply with all of its requirements.

Participant understands that Camp Valle Verde reserves the right to dismiss any person from further participation in the Camp Session in the event that its representatives shall determine that such person has been guilty of a violation of the code of conduct in the sole judgment and opinion of Camp Valle Verde. If this shall occur, any additional transportation costs resulting from the early return home of such participant shall be borne by and be the responsibility of the participant or his/her parent or guardian.

Participant releases Camp Valle Verde, its representatives and all other persons in charge of activities in their individual capacities from liability for personal injury resulting from the failure of camper or other camp participants to obey safety regulations and directions of activity leaders or resulting from the exercise of judgment by such activity leader made in good faith in response to emergencies that may occur from time to time. Nothing herein shall excuse any of such representatives or activity leaders from the responsibility to act with reasonable care for the safety of all participants during the course of activities and events.

Participant understands that neither Camp Valle Verde nor any of its representatives shall be liable for loss or damage to the personal property and possessions of the participant in the absence of willful misconduct by such persons.

Any medical costs incurred on behalf of participant that exceeds the insurance coverage obtained by Camp Valle Verde shall be the sole responsibility of participant and/or his/her parent or legal guardian. Camp Valle Verde has permission to provide or obtain medical attention for the participant in the event of sickness or injury. The physician selected by the Camp Director or General Coordinator has permission to hospitalize, secure proper treatment for and order injections and/or anesthesia and/or surgery for the participant. Camp Valle Verde Health Staff may dispense over-the-counter medications where deemed necessary.

Participant and/or his/her parent or legal guardian shall be liable for any damage to the property or facilities of the Camp or the property or facilities of others resulting from the acts of participant, either solely or in concert with others.

Participant consents to the taking and use of any slides, photographs or videotaping during the Camp Session, whether for advertising, promotion and publicity purposes by Camp Valle Verde and waives all claims for any compensation for such use or for damage.
To qualify to take part in this Camp Session, participant and/or parent or legal guardian has submitted all information required in instruments like the camper application and helth form and has signed this Participation Agreement.

Participant represents that all of the information contained in such instruments is true and correct and that Camp Valle Verde and its representatives have full right and authority to rely on information contained therein. Participant further recognizes that Camp Valle Verde reserves the right to reject any participant in the event of the failure or refusal of participant and/or parent or legal guardiana to sign and execute all of the foregoing required documents.
